Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care without regard to ability to pay. The Cancer Screening Patient Navigator is a contracted position funded by the Delaware Division of Public Health. The Cancer Screening Patient Navigator will guide individuals through the cancer-screening process. This includes, but is not limited to, educating eligible individuals about cancer screening and associated processes, scheduling screening appointments, linking patients with cancer screening programs such as Screening for Life, and eliminating barriers to screening. The Cancer Screening Patient Navigator may also assess patient health problems and needs, maintain medical records and advise patients on health maintenance and disease prevention, and act as a clinical resource for the site Office Manager.

RESPONSIBILITIES
- Identify patients in need of cancer screening and guide them through the cancer screening process.
- Provide education and support regarding the need for cancer screening.
- Serve as an advocate for patients and providers by determining and meeting their needs as well as underscoring the importance of screening and being active participants in their own healthcare.
- Coordinate services and support to reduce patient barriers to cancer screening and care
- Facilitate evidence-based interventions including patient reminder activities.
- Track and monitor interventions and outcomes and report data as required.
- Participate in internal and external meetings, including meetings with the Division of Public Health (DPH) or its designee(s) to review success and challenges and participate in the development and implementation of actions to improve cancer screening rates.
MINIMUM OBJECTIVE QUALIFICATIONS
- Registered Nurse or a Licensed Practical Nurse with current Delaware licensure or Medical Assistant certification by a national organization accredited by the National Commission for Certifying Agencies (NCCA) or the American National Standards Institute (ANSI).
- At least 1 year of experience with health promotion, disease prevention, and health education in a community health setting.
- Experience using computers, Microsoft Word, Excel, and Outlook and electronic medical records software
- Fluency in Spanish strongly preferred
