Designated as the Social Care Network (SCN) Lead Entity for the Capital Region, Central NY, and North Country under New York’s 1115 Waiver Amendment’s SCN & Health-Related Social Needs (HRSN) Program, we are responsible for ensuring that there is a seamless, consistent end-to-end process for HRSN screening, navigation, and the delivery of HRSN and other services to thousands of Medicaid Members. Who You Are
The Performance Manager is responsible for providing day-to-day oversight of the Performance team to ensure Healthy Alliance’s successful implementation and execution of terms identified under New York’s 1115 Waiver Amendment’s SCN & HRSN Program and contracts; the Quality Plan; and organizational performance of partners in meeting Network Standards. This role ensures community members and partners are well supported for network optimization and growth.

What You’ll Do
- Oversee and direct the daily functions of Performance Consultants (PCs), as defined by an established Internal Quality Committee (IQC) Quality Plan and Network Standards.
- Manage partners, including but not limited to assisting training activities for new and existing user groups to support network adoption, optimizing platform usage and contract management activities for members and network partners, and supporting remediation efforts for any identified improvement opportunities.
- Engage with partners in a consulting role to understand their current processes and make recommendations for areas of improvement, in collaboration with PCs and Cross-Functional teams, to optimize use of the platform and support network health.
- Collaborate on strategic opportunities with other members of the Leadership team to further enhance service delivery excellence.
- Collaborate with external stakeholders support of contract deliverables, including but not limited to Managed Care contracts and other funding sources.
- Review opportunities and offer solutions to expand service types by geography.
- Work with the Network Development team to recruit organizations to fill service gaps.
- Coordinate product demonstrations and fact-finding meetings for interested agencies in collaboration with Network Development and Referral Coordination Center (RCC) teams as needed.
- Maintain a pipeline of potential Healthy Alliance partners and regularly report status, including announcing new partners, partner successes, and other partner updates in the monthly newsletter to inform the network of ongoing growth.
- Evaluate partners’ effective use of the Healthy Alliance Integrated Network (ecosystem and SCN) and Services through collaborative quality improvement reviews with the RCC Manager, Lead Navigator, and Lead PC.
- Host regularly scheduled meetings and training sessions in collaboration with the Performance team.
- Analyze partner-level performance data for trends and opportunities, providing recommendations to our Performance and Network Development teams.
- Assess and report on partner effectiveness and efficiencies to the Performance Director and Leadership team.
- Monitor partner-level adherence to Network Standards, compliance with established IQC Quality Plan, New York’s 1115 Waiver Amendment’s SCN & HRSN Program requirements, and contracts.
- Maintain consistent communication with the Leadership team regarding potential risks that require further engagement or escalation.
- Develop and maintain materials/collateral to support effective partner communication.
- Demonstrate a customer-first mindset and presence on all partner and stakeholder?interactions.
- Learn and stay current on Healthy Alliance’s Solutions Suite.
- Work collaboratively with other internal teams as well as Unite Us Account Managers and other critical vendors.
