About the Role
The Recovery Partner (RP) will serve as an extension of the Recovery Care Coordinator (RCC) and Social Services Coordinator (SSC) in patient care management. The Recovery Partner will be responsible for carrying out the plan of care created by the RCC/SSC. The goal of the RP will be to reinforce the patient’s confidence in self-management of their care and ensure that the necessary transition of care tasks are completed before the patient leaves Contessa Services. This position is a part-time weekend role (approx. 16 hours) and requires flexibility on weekends. This position will work from home once trained.
