We are seeking a highly motivated and compassionate Care Coordinator to join our team. The Care Coordinator will be responsible for ensuring that our patients receive the highest quality of care and support throughout their treatment journey.
Responsibilities
- Handle External/Internal referrals.
- Provide exceptional patient care, including the rooming of patients when needed.
- Coordinate and schedule surgeries, as well as follow-up appointments as designated by physicians.
- Organizing, reviewing with patients when asked to and obtaining patient signature, and sending surgery packets to Surgery Schedulers.
- Maintaining physician surgery scheduling book/calendar.
- Check, document, and return calls from voicemails.
- Daily follow up communication with patients.
- Daily check of the patient portal.
- Clinic chart prep (review chart daily for visit) and scrub schedules.
- Ensure all patient testing is in the chart before patient’s appointment (i.e., MRI, CT, Labs).
- EMG referrals and following up with results.
- Track all test results, schedule respective follow ups, and close the loop.
- Communicate with patients regarding medication refills once they have been sent to the pharmacy by the MA or physician. Document and close the loop.
- Obtain Prior Authorizations for XIAFLEX.
- Minimum High School Graduate or GED.
- Strong communication and interpersonal skills, with the ability to build rapport with patients and their families.
- Excellent organizational skills and attention to detail.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office with the ability learn an Electronic Medical Record.
