Description
The Stabilization Case Manager is responsible for providing clients with a successful transition into permanent housing. This individual will work closely with clients and help them build independence and continued stability.
Responsibilities (including but not limited to):
- Develop stabilization plans and record necessary information in client records.
- Conduct a needs assessment to determine housing goals and objectives.
- Collaborate with other departments as needed to ensure client’s needs are met.
- Perform ongoing case management with clients and meet with them at least once a week, or as needed.
- This may include assisting with daycare, transportation, or financial arrangements.
- Maintain case files and ensure database is current.
- Complete re-assessments per company standards and requirements.
- This individual may need to provide clients with transportation to and from appointments.
Requirements
- Bilingual, highly preferred.
- Bachelors degree and/or at least two years of Human Services experience.
- Strong case management skills.
- Experience working with low-income families.
- Excellent written, verbal, and interpersonal communication skills.
- Must have reliable transportation.
