Stabilization Case Manager
Description

The Stabilization Case Manager is responsible for providing clients with a successful transition into permanent housing. This individual will work closely with clients and help them build independence and continued stability.


Responsibilities (including but not limited to):

  • Develop stabilization plans and record necessary information in client records.
  • Conduct a needs assessment to determine housing goals and objectives.
  • Collaborate with other departments as needed to ensure client’s needs are met.
  • Perform ongoing case management with clients and meet with them at least once a week, or as needed.
  • This may include assisting with daycare, transportation, or financial arrangements.
  • Maintain case files and ensure database is current.
  • Complete re-assessments per company standards and requirements.
  • This individual may need to provide clients with transportation to and from appointments.
Requirements
  • Bilingual, highly preferred.
  • Bachelors degree and/or at least two years of Human Services experience.
  • Strong case management skills.
  • Experience working with low-income families.
  • Excellent written, verbal, and interpersonal communication skills.
  • Must have reliable transportation.
