Your Role: Under the direction of an RN, you'll provide nursing care and treatment to elderly, acutely ill, or injured residents, integrating technical skills and theoretical knowledge to meet their individual needs.


What You'll Do:

  • Direct Care: Provide direct care and assist with care as directed.
  • SBAR Process: Collaborate with the RN for assessments and recommendations.
  • Rounds: Make rounds with physicians and other team members.
  • Care Planning: Participate in resident evaluations and care planning activities.
  • Coordination: Coordinate residents' care activities and communicate changes to team members and families.
  • Medication Management: Oversee medication administration and administer treatments.
  • Documentation: Document care performed and observations of resident status promptly.
  • Emergency Response: Assist with emergencies and administer cardiopulmonary resuscitation.
  • Clinical Admission: Complete clinical admission paperwork for new admissions.
  • Discharge Process: Assist with the clinical discharge process.
  • Punctuality: Ensure regular attendance for assigned shifts.

Skills & Abilities:

  • Emergency Response: React decisively and quickly in emergencies.
  • Effective Communication: Communicate effectively with residents, families, and staff.

Requirements

Education

Graduate of accredited LPN School of Nursing.


Licenses/Certification  

Current valid LPN license in the state employed.

Valid CPR certification.


Experience

Six months experience in a long-term care environment preferred.
