Job Title: Life Skills Coach
Job Summary:
We are seeking a highly motivated and experienced Life Skills Coach to join our team. The Life Skills Coach will be responsible for providing guidance and support to individuals in developing essential life skills that will enable them to achieve their personal and professional goals. The ideal candidate will have a passion for helping others and a proven track record of success in coaching and mentoring.
Responsibilities:
- PCISP & Behavior Support: Actively participate in the development and implementation of Person-Centered Individualized Support Plans (PCISP) and/or Behavior Support Plans.
- Community Integration: Support individuals in engaging with their community to foster skill development, independence, and socialization. This includes self-care, sensory-motor development, daily living skills, communication, and social skills.
- Safety & Well-Being: Maintain a safe and supportive environment for the individual.
- Personal Care Assistance: Help with mobility, transfers, bathing, shaving, grooming, oral hygiene, toileting, dressing, feeding, and monitoring nutritional intake.
- Daily Living Support: Assist with:
- Meal planning, preparation, and cleanup
- Shopping and errands
- Employment or volunteer training and skill application
- Household chores and leisure activities
- Transportation needs
- Correspondence and bill paying
- Homemaking tasks to ensure a clean, safe, and healthy living environment
- Medication reminders or assistance
- Other Duties: Perform additional responsibilities as needed to support the individual's well-being and independence.
Reports to: Director of Services, Team Leads & Executive Director

Age & Technology:
- Must be at least 18 years old.
- Must have a smartphone or tablet with Wi-Fi for email and note-taking.
Licensing & Certifications:
- Must have and maintain a valid driver’s license and auto insurance if transporting clients or driving on company business.
- Must have a current CPR certification, renewed at least every two years.
Health & Background:
- Must pass a criminal history background check.
- Must have a negative TB screening before starting services and update as required.
Skills & Abilities:
- Must be able to communicate effectively to complete required forms and reports and follow oral or written instructions.
- Must be able to provide services according to an individual’s Person-Centered Individualized Support Plan (PCISP).
- Must be willing to accept supervision.
- Must demonstrate interest in and empathy for individuals receiving support.
