About the Position:
The Clinical Assessment Coordinator provides professional assessments of individuals and families for program services and serves as a liaison to the All Points North Programs. Provides screening, intake and care coordination support and leadership to contact center staff and consumers seeking services at APN. Provides support and leadership to the overall operations including Community Resource team.
Position Duties and Responsibilities – Responsibilities include but are not limited to:
Assessment:
- Represents All Points North in a professional manner to local, regional, and national stakeholders.
- Assists with client assessment and decision making for admission to program.
- Coordinates screening, triage individual needs, appropriate level of care recommendations and admission with follow-up to referral sources.
- Maintains CRM database and integration with EHR.
Admissions:
- Provides screening, scheduling and intake to consumers and referral sources. May include off-site work at various APN facilities.
- Maintains tracking log to monitor disposition of consumer referrals.
- Provides facility and program orientation to consumers entering services.
- Provide information for utilization management and review.
- Provides data input into the electronic health record system.
- Manages client data for input and discharge planning within EHR.
- Works with revenue cycle management team to coordinate data for claims payment.
- Develops, monitors, and manages tracking systems for client data.
- Establishes and maintains positive working relationships with contact center staff and community resource team, and referral sources.
- Represents All Points North Lodge’s services in a professional manner with community stakeholders.
- Maintains timely and accurate filing for clinical records.
- Assists with procurement of resources for organizational deliverables.
- Provides clinical support to the for all pre-admission team.
- Responsible for working in conjunction with contact center staff, to support the mission of All Points North
- Complete all initial and annual required training.
- Responsible for other duties as assigned by the supervisor.
Qualifications: The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Education: This position requires a minimum of a master’s degree in behavioral science, or related discipline.
Experience: Minimum of two years' experience in an addiction and\ or mental health residential-outpatient treatment setting and integration with a community resource team. Prefer License Professional in Behavioral Health or Certification in Colorado as a CACII or higher.
Special Skills & Knowledge: Candidates should have the following skills and attributes: must be provide professional assessment and work collaborative with contact center team; able to communicate well with potential people in need of professional services; be adaptable to meet the organizational needs during growth and accept changes quickly and easily; be willing to address conflict appropriately. Basic computer skills are required with Microsoft Office Suite and web-based products knowledge a bonus.
