Job Description Summary
The Hospice LVN is an integral part of the hospice clinical team. Under the direction of the Director of Nursing, the job responsibilities include the following:
Key Responsibilities:
- Patient Care:
- Provide direct nursing care to hospice patients in their homes, residential care facilities, or hospice facilities.
- Administer medications and treatments as prescribed by the physician.
- Monitor patients' conditions and report changes to the RN Case Manager or physician.
- Assist with personal care tasks such as bathing, dressing, and grooming.
- Assessment and Documentation:
- Conduct patient assessments to evaluate physical, emotional, and psychosocial needs.
- Maintain accurate, timely, and thorough documentation of patient care and observations in the electronic health record (EHR) system.
- Participate in the development and implementation of individualized care plans.
- Collaboration and Communication:
- Communicate effectively with patients, families, and caregivers to provide education and support.
- Collaborate with interdisciplinary team members, including physicians, social workers, chaplains, and volunteers, to coordinate care.
- Attend and participate in team meetings, case conferences, and continuing education programs.
- Compliance and Quality Assurance:
- Adhere to all hospice policies, procedures, and regulatory requirements.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Ensure compliance with infection control practices and safety standards.
Qualifications:
- Education: Graduate of an accredited vocational nursing program.
- Licensure: Current, active LVN license in the state of [State].
- Experience: Minimum of one year of nursing experience, preferably in hospice, palliative care, home health, or a related field.
- Skills:
- Proficient in clinical nursing skills, including pain and symptom management.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate, patient-focused approach to care.
- Proficient in electronic health record (EHR) systems.
Physical Demands:
- Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
