 Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care without regard to ability to pay. The Rural Health Outreach Ambulatory Care Nurse Supervisor helps to manage clinical service delivery for patients in the rural communities of Kent and western Sussex Counties. The Rural Health Outreach Ambulatory Care Nurse Supervisor will work in collaboration and partnership with the Rural Health Outreach Program Manager to deliver results for the Rural Health Outreach Program. Duties include outreach and facilitating clinical care, ensuring smooth operation of the clinical team on the Mobile Health Unit (MHU) and through a mobile outreach van, initial preparation of the patient, patient education, assisting the provider during patient examinations, phlebotomy services, Direct Observation Therapy, and case management. The Rural Health Outreach Ambulatory Care Nurse Supervisor will manage the Rural Health Outreach Medical Assistant.

Responsibilities of Nurse Supervisor


  1. Engage in outreach initiatives in collaboration with the Rural Heath Outreach Program Manager as directed by the Associate Director of Clinical Operations and Director of External Affairs. 
  2. Maintain the Mobile Health Unit (MHU) in accordance with Joint Commission standards, including stocking and maintenance of diagnostic equipment, supplies and medications needed for patient care. Perform and document daily completion of QC logs and maintain POCT before beginning patient care.
  3. Assist providers and medical assistants with patient care, e.g., take vital signs, perform EKGs, collect specimens, irrigate ears, dress wounds, splint and tape injuries, perform selected diagnostic tests, and assist with pap smears, physical exams, and minor procedures when necessary.
  4. Oversee clinical care on the MHU and out in the community to ensure culturally competent, high quality, evidence-based care in accordance with Westside’s standards, policies, and procedures.
  5. Work with the Nurse Manager and Clinical Floor Supervisor to complete the weekly staffing schedule, clinical services delivered in the community, monitor attendance and adequately communicate all last-minute schedule changes to all affected personnel.
  6. Proactively and continuously reviews patient medical records to identify any care needs requiring intervention.
  7. Educates patient/family about medical conditions (existing or newly diagnosed) to assist them in appropriate self-management.
  8. Attend community meetings and participate in community events/fairs as directed
  9. Maintains compliance with all professional and organization requirements to meet the positions standards
  10. Lead team outings and outreach during nurse led health screening events
  11. Support Westside’s Dover Health Center when not on the Mobile Health Unit
Requirements

MINIMUM OBJECTIVE QUALIFICATIONS

  1. State of Delaware (or compact state) Registered Nurse licensure with no restrictions or disciplinary action
  2. Minimum of 2 years’ experience in the area of ambulatory care or comparable
  3. Current certification in Basic Life Support for Healthcare Providers
  4. Become trained in phlebotomy within six months of hire
  5. At least six months of experience with electronic medical record software
  6. Ability to travel independently to locations (reliable transportation, insured vehicle and valid driver’s license)
  7. Experience using Microsoft Word, Excel and Outlook

PREFERRED OBJECTIVE QUALIFICATIONS

  1. Fluency in written and spoken English and spoken Spanish, including knowledge of medical terminology
  2. Clinical experience, training and/or passion to provide quality care to underserved populations
  3. Leadership/Program Development Experience
  4. Ambulatory Care Certified
