What You’ll Do
- Complete in-depth clinical assessments and develop individualized care plans
- Provide caregiver training on medication safety, chronic disease management, and home-based support
- Partner with IDT members to coordinate wraparound care
- Participate in rotating on-call coverage for clinical needs
- Document Participant progress and care interventions clearly and consistently
Where You’ll Work
Your day may start at one of our welcoming Day Health Centers in Greentree, Northshore, Allegheny Center, or McKees Rocks, but your impact reaches far beyond. Our nurses travel throughout Allegheny County to deliver care where it matters most—in the community, in homes, and in the hearts of those we serve.
You’ll serve older adults throughout Allegheny County, providing care at our Day Health Centers and in Participants’ homes, community spaces, or local healthcare facilities—wherever support is needed most.
What You’ll Need
- Active PA Registered Nurse license
- Minimum one year of experience working with older adults or medically complex individuals
- CPR certification
- Valid driver’s license, reliable vehicle, and auto insurance
