We are looking for a compassionate, dependable, and team focused Licensed Practical Nurse (LPN) to join our nursing team. In this role, you will provide hands on care, support residents and families, collaborate with the clinical team, and help create a positive and healing environment for those we serve.

What You’ll Do
• Provide direct nursing care and support to residents and patients
• Administer medications and treatments safely and accurately
• Monitor resident conditions and communicate changes to the care team and physicians
• Assist with assessments, care planning, and resident evaluations alongside the RN team
• Complete charting, documentation, and clinical records in a timely and accurate manner
• Support residents and families with compassion, professionalism, and respect
• Assist with admissions, discharges, and care coordination
• Respond to emergencies and provide CPR when necessary
• Help oversee Medication Aides and treatment administration when applicable
• Maintain a clean, organized, and safe clinical environment
• Follow all federal, state, facility, and company regulations and nursing standards
What We’re Looking For
• Current LPN license in the State of Oregon
• Valid CPR certification
• Strong clinical and critical thinking skills
• Ability to stay calm and organized in a fast paced environment
• Compassionate, patient centered approach to care
• Strong communication and teamwork skills
• Dependable attendance and punctuality
• Strong documentation and organizational skills
• Long term care or skilled nursing experience preferred, but not required

Education
• Graduate of an accredited LPN School of Nursing
Licenses/Certifications
• Current valid LPN license in the state employed
• Valid CPR certification
Experience
• Six months of experience in a long term care environment preferred
