New Competitive Starting Rate of $47,500 -$50,000 Annually. 

Please note, this is an hourly rate of $22.84 - $24.04 per hour, and there may be opportunities for overtime!!


Threshold Rehabilitation Services, Inc., a human service agency supporting people with disabilities, is actively seeking energetic, team-oriented professionals to join our management team in our IDD Residential Services Division as a Program Specialist- Community Homes.


Responsibilities include:

  •  Supervision of direct support employees.
  •  Document staff supervision as required.
  • Ensure complete and accurate data regarding clients is updated in EHR System.
  • Assist with intake interviews and complete necessary paperwork.
  • Maintain and monitor all client documentation to ensure accuracy.
  • Ensure adherence to operational policies and procedures.
  • Complete Day 1 orientation and paperwork for all admissions to the program.
  • Assist individuals in accessing and utilizing community resources, accessing medical and financial resources.
  • Develop service plans and reviews.
  • Assist with community integration.
  • Help establish and maintain a therapeutic environment.
  • Communicate and enforce state regulations, agency contracts, directives, and contract expectations.
  • Monitor each residence to ensure that all assigned programs are managed within full compliance with state regulations and agency contracts.
  • Serve as a liaison with families and other provider agencies as assigned.
  • Complete weekly attendance and monthly reports.
  • Provide effective team leadership.
  • Schedule staff to ensure assigned programs are covered.
  • Ensure the facility meets physical site standards and ensure need repairs are completed.
  • Complete all incident/accident reports in compliance with state regulations, agency contracts, and directives of the Director or Assistant Director of IDD Residential Services.
  • Comply with Threshold's confidentiality policy specifically regarding access to individual's personal health information.
  • Perform other related duties as assigned.

The hours for this position are Monday to Friday, 8am - 5pm, flexible, with on-call responsibilities.  These are great work hours with paid holidays off starting day one of employment!!

Requirements
  • A bachelor's degree in a special education, Psychology, Public Health, Rehabilitation, Social Work, Speech Pathology, Audiology, Occupational Therapy, or Therapeutic Recreation. (Original document is required).  
  • At least two years of experience working directly with individuals with intellectual disabilities.
  • Two years managerial/supervisory experience, preferred.
  • Computer knowledge is a must.
  • Valid PA Driver's License.
  • PA Motor Vehicle Record Check.
  • Use of vehicle while working with valid registration and insurance coverage indicating you are an insured driver.
  • Pre-employment Drug Screen.
  • Pre-employment Physical Exam.
  • Pre-employment TB (Mantoux) Test or Chest X-Ray (if applicable).
  • PA Criminal History Clearance.
  • FBI Clearance (if candidate has not been a resident of PA for the past two consecutive years).
