Clinical Social Worker -Foster Care
Description

Job Description. The Clinical Social Worker (CSW) facilitates foster care placement by providing services to children requiring foster placement and to their resource families. The CSW conducts supervised visits, assessments and develops individual case plans and treatment objectives for each client to facilitate successful foster care placement. The CSW may utilize bilingual skills as needed. The CSW will consult with the CSWS and Program Director to ensure quality and ethically sound services that also maintain compliance with State, AGH and CARF Accreditation regulations. CSW IV participated in on-call rotation to support families 24/7. 


Key Performance Areas.

  • Provides individual, group and family case management and counseling and supportive services. 
  • Make regular home visits to approved resource families to assess and/or address the needs of children and to provide support to resource parents.
  • Develop individual case plans and treatment objectives for children.
  • Work closely with AGH Mental Health Services Department and other Mental Health service providers to coordinate all services and arrange appropriate referrals to meet treatment objectives.
  • Monitor that treatment objectives are met. 
  • Develops a comprehensive psychosocial assessment.
  • Conducts assessments and screens as required, depending on the program.
  • Participate in and provide support groups and classes to the community and clients of the agency
  • Develops and maintains effective working relationships with participants, families and significant others. 
  • Participate in case conferences, support groups, in-service training, and other agency functions.
  • Provide support through transition planning.
  • Assess families’ strengths & needs and provide in-home training to address areas of concern.
  • Coordinate intake and placement of children, with other professionals, to best address the child’s well-being and permanency while in care.
  • Responds to the needs of the public and the participants in a service friendly manner that results in screening, intakes and referrals for appropriate AGH services.
  • Must be available and able to provide services when clients and families need them, which can include evenings and weekends in order to ensure coordination and continuity of services.
  • Transport clients to appointments and activities as required. 
  • Develops and maintains networking relationships within the agency and with other professionals and members of the community who can provide program resources.

Starting Salary: $ $70,304

Qualifications. A successful candidate must have the following skills and experience.

  • Master’s Degree in Social Work, Counseling or a related field recognized by Community Care Licensing or Education and experience equivalent to that qualifies for CCL Exception. 
  • Maintain a welcoming environment for families and other community partners.
  • Ensure compliance with HIPAA privacy practices.
  • Must pass and remain current in CPR/First Aid. 
  • Must be able to effectively utilize computers and basic computer software. 
  • Must be available to work evenings, weekends and some holidays as needed. 
  • Must meet Community Care Licensing and insurance requirements. 
  • Must have a valid California Driver’s License and a good driving record. Must be at least 18 years old with at least 2 years of driving experience. 
  • A Greater Hope includes all employees in the DMV pull notification process, notifying AGH of infractions throughout the process. 
  • All Employees are required to report all arrests within 48 hours of arrest as per Community Care Licensing requirements. 
  • Must pass a drug screen, physical exam, and Tuberculosis skin test. 
  • Be able to obtain Criminal and Child Abuse Clearance from the State of California/Department of Justice.
