A Day in the Life of a Hospice Liaison
As a Hospice Liaison primarily for the DeKalb/Sycamore area but will also provide service in the Rockford and surrounding areas, your day blends purpose with strategy. You will meet with physicians, discharge planners, and community partners, using your hospice knowledge and RN perspective to educate them on services and eligibility. You’ll collaborate closely with leadership on marketing initiatives, adapt outreach strategies, and identify new opportunities to serve patients sooner. As an RN, your clinical credibility strengthens every conversation.
Hospice Liaison Responsibilities:
- Execute daily sales and marketing strategies to referral sources
- Serve as a knowledgeable liaison between providers and hospice services
- Educate healthcare professionals and the community on hospice care
- Build and sustain referral relationships using RN clinical expertise
- Develop referral growth plans with the Marketing & Community Relations Director
- Stay current on hospice regulations and market trends
- Represent Northern Illinois Hospice with professionalism and heart
Requirements:
- Associate degree required (ADN); BSN preferred
- Professional Registered Nurse License in the state of Illinois
- Three (3) years nursing experience in a hospital or facility setting.
- Hospice or palliative care experience preferred; marketing experience preferred
- Strong communication, presentation, and relationship-building skills
- Knowledge of hospice regulations and community healthcare networks
- Valid drivers’ license, current auto insurance and access to a reliable automobile.
Schedule: Full-time, Monday – Friday (daytime hours)
Salary Description
We support our employees’ wellbeing and regularly benchmark with other companies in our industry to offer an extensive Total Reward package for this position. The expected base salary range for this role is $79,000 – $114,000. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.
