1.0 INTRODUCTION

1.1 The purpose of this standard procedure is to outline the duties, responsibilities, authority, reporting relationships, and measures of performance for the position of CNA/Direct Support Professional in the Adult Day Health Care program.

2.0 BASIC FUNCTION

2.1  The basic function of the CNA/DSP is to provide quality social, recreational, vocational and basic community skills as well as personal care for Adult Day Health Care participants under the direction of the Manager of the Adult Day Health Care. The CNA/DSP is also responsible for the daily and monthly documentation in accordance with SCL, MPW, and HCB waiver regulations. 

3.0 REQUIREMENTS:

3.1 Education:

· Required: High School diploma or GED

· CNA certification required

· Meet all requirements to drive agency vehicles.

3.2 Experience:

One or more year’s related experience with individuals in a health care or social service setting is preferred.

One or more year’s working with individuals with developmental disabilities preferred. 

3.3 Skill, Knowledge and Abilities:

· Knowledge of the Adult Day Health care services being provided.

· Ability to volunteer for various activities throughout the year. 

 

3.4 Physical:

Ability to physically observe and perform operations (lifting, bending, and stooping are required), listen and actively participate in communicating effectively with coworkers, participants, caregivers and public.

· Required to lift, carry, push or pull between 25 to 50 pounds.

· Requires sitting, standing, walking, bending, squatting, kneeling, stooping, and reaching.

· Requires driving and assisting individuals in and out of vehicles.

· Requires transferring individuals in and out of wheelchairs.

· Requires operating a mechanical lift. 

3.5 Mental:

Basic statistical and analytical business math skills are required. The ability to read, write, spell and communicate clearly and effectively in the English language on all technical and business levels is required. Basic computer knowledge and ability is required. 

3.6 Must successfully pass all criminal background and abuse checks

3.7 Must have a valid driver’s license and insurance.

4.0 REPORTING RELATIONSHIPS

4.1 The CNA/DSP will report directly to the nursing staff of the ADHC program, Assistant Manager and the Manager of Adult Day Health Care. 

4.2 The following positions report directly to the Direct Support Professional/CNA; None

5.0 AUTHORITY

5.1 To assist the Manager and Assistant Manager of ADHC in developing, implementing and maintaining the ADHC program contacts. 

5.2 To request from the Manager and Assistant Manager of ADHC the purchase of equipment, materials, operating supplies and services within the guidelines established by the approved budget. 

5.3  Commit company resources as directed by the Manager and Assistant Manager of Adult Day Health Care.

6.0 RESPONSIBILITIES

6.1 Maintain an effective and efficient operation in compliance with all applicable laws and regulations.

6.2 To act as a liaison between the organization and the agencies and or guardians who refer participants to the Adult Day Health Care program. 

6.3 Ensure a safe work environment.

6.4 Ensure that quality standards are adhered to.

6.5 Support and enforce all company policies and procedures.

6.6 Maintain open communication with the Manager and Assistant Manager of Adult Day Health Care, Nurses, Vice President of Adult Services and all other employees.

6.6 Maintain necessary documentation, within the required timeframe, on all participants on their caseload.

6.7  Ensure all documentation follows guidelines set forth by SCL, MPW, and HCB as well as all other governing entities. Provide the VP of Adult Services with the weekly art schedule, monthly art schedule, and scheduled fundraising events.

6.8  To gather information for the Manager and Assistant Manager of Adult Day Health Care to help prepare appropriate budget and service information and  disseminate to the Vice President of Adult Services.

6.9  Recommend to the Manager and Assistant Manager of Adult Day Health Care the purchase of required Capital equipment.

6.10 Control operational costs and expenses.

6.11 Preserve and maintain the physical assets of the company by providing adequate 

Security and ensure that controls are established and enforced. 

6.12 Ensure that all participants are provided with a meaningful day.

6.13 Contribute to a positive work environment. 

6.14 Participate in various volunteer activities throughout the year.

6.15 Treat all participants and coworkers with dignity and respect at all times. 

7.0 PRINCIPAL DUTIES

7.1 Assist the Manager of Adult Day Health Care in developing, implementing and 

maintaining the controls necessary to meet corporate and Adult Day Health 

Care program goals.

7.2 Provide personal care and assistance as required by the participants of the Adult

Day Health Care program.

· Assist participants with activities of daily living (toileting, bathing, ambulating, transfers, feeding, changing clothes, etc.)

· Report indications of possible illness immediately to the center nurse.

· Assist participants who become ill while at the center.

· Support the nurse in obtaining vital signs of participants.

· Respect participants rights.

· Follow HIPPA confidentiality guidelines. 

7.3 Assist with the preparation, service and feeding of breakfast, lunch and snacks. 

7.4 Assist with participants boarding and exiting vans (use of mechanical van lift).

7.5 Plan and implement activities and classes based on the participants’ plan of care.

· Use allotted planning time effectively.

· Use allotted preparation time effectively. 

· Implement classes, outings, and craft activities as assigned. 

7.6 Model and promote respectful interactions with participants

· Interact with participants in a respectful, empathetic, age appropriate and culturally appropriate manner. 

· Respect participant’s preferences and request, based upon plan of care information. 

· Respect all committee approved Rights Restrictions or the lack there of.

· Use appropriate tone and voice at all times.

7.7 Maintain a neat and clean program area

· Clean program areas on a daily basis (dusting, cleaning windows and doors, taking out trash, sweeping and mopping, and any other duty necessary to keep the areas clean and sanitary)

· Clean / disinfect program areas and supplies daily

· Clean up spills/ bodily fluids using appropriate infection control procedures

· Check and replenish supplies

· Promote a clean and sanitary environment

· Wear gloves when cleaning with chemicals and when providing personal care.

7.8 Monitor and respond to safety of participants

· Ensure the safety of each participant by participating in safety training and emergency drills

· Complete and maintain certification in CPR, First Aid and Universal Precautions

· Know emergency procedures and implement as necessary

· Report suspected abuse, neglect and exploitation of participants immediately per Easter Seals policies and procedures

7.9 Participate as a member of the interdisciplinary team, providing appropriate 

Information for participants plan of care. 

7.10 Perform other duties as assigned, including attending required training sessions.

7.11 Work with management to ensure company policy is followed.

7.12 Provide community experiences for the Adult Day Health Care participants.

7.13 Control expenditures to conform to budgetary requirements. 

7.14 Report suspected abuse, neglect, and exploitation of participants immediately per 

Easter Seals policies and procedures.

7.15 Council with Adult Day Health Care participants as needed.

7.16 Complete any other duties assigned by the nursing staff and program manager.

7.17 Assist in preparing for annual Medicaid and licensing review of the ADHC 

program. 

7.18 Act as a liaison between referring agencies, families, participants and employees 

and as an advocate for the ADHC participants.
