Primary Job Objective/Job Summary:


The Resource & Referral Coordinator will play a pivotal role in enhancing the accessibility and coordination of services across South Carolina. This position is primarily responsible for developing and managing a comprehensive referral database and supporting the referral process for providers and families. A portion of this position is grant funded.

Essential Functions:


SC Thrive Knowledge:

  •  Become a trained SC Thrive Counselor with the ability to perform client intake interviews and complete applications as well as determine how to expand and best implement usage of Thrive Hub within partner agencies.
  •  Become fully knowledgeable and able to present at numerous speaking engagements on the breadth of SC Thrive work.
  •  Participate in high leverage state and national coalitions, events, conferences, etc. to promote the work of SC Thrive.
  •  Become knowledgeable in all aspects of SC Thrive’s client management system Thrive Hub and all other SC Thrive trainings.
  •  Stay current on industry and organization trends, products, competitive activity, and customer needs.
  •  Coordinate all aspects of the SC Thrive service directory and needed resources, including research, data entry, and updates.
  •  Work with internal and external stakeholders to help provide support and insight for SC Thrive systems and resources.
  •  Conduct routine reviews of published service listings to ensure accuracy, consistency, and alignment with current provider availability and service details.
  •  Track, monitor, and document referrals to support follow-up efforts and help ensure clients are able to access services whenever possible.
  •  Respond to inbound calls and inquiries, assessing client needs and providing accurate, timely referrals to appropriate services and resources to support successful outcomes.
  •  General reports on the availability, utilization, and updates of resources to key stakeholders.
  •  Other duties as assigned.
Requirements

Qualifications and Experience Required:

  •  Bachelor's degree in Public Health, Social Work, Information Technology, or a related field
  •  1–3 years of experience in resource navigation, referral coordination, customer service, call center support, case management, or a related human services role.
  •  Ability to build and maintain partnerships with external organizations
  •  Possesses strong research skills and the ability to conduct internet navigation and research to assist clients
  •  Displays strong attention to detail, problem solving, analytical and organizational skills
  •  Experience using web-based systems or databases to enter, update, and retrieve information accurately and efficiently.

Preferred Qualifications and Experience:

  •  Experience working in nonprofit, public sector, or community-based organizations.
  •  Experience maintaining or working with a service directory, resource database, or referral platform.
  •  Prior experience tracking referrals, conducting follow-ups, or documenting outcomes to support service coordination.

Additional Notes/Requirements/Qualifications:

  •  Pre-employment screens are required per standard policy to include background and drug screening.
  •  Ability and willingness to work flexible hours, including some nights and weekends, as needed.
  •  Must have reliable transportation or be able to secure transportation for travel as needed.
  •  Must have a valid SC driver’s license, minimum state auto insurance, and the ability to travel to multiple worksites in SC.
  •  Must maintain an acceptable driving record in accordance with SC Thrive policy.
  •  Must have consistent, secure, high-speed internet access that is at least 25 Mbps download and 4 Mbps upload and meets SC Thrive’s standards.

Working Conditions and Physical Demands:


The job will usually be performed in a standard office environment, but occasionally, travel will be required. Occasional work outside standard business hours, including nights, overnights and weekends, may be required for special projects and events. Occasionally travels and works in outdoor weather conditions.


Must be able to remain stationary 70% of the time. The individual in this job must occasionally move about inside the office to access filing cabinets, office equipment, etc. Constantly operates a computer and other office productivity equipment, such as calculators, printers, etc. Frequently positions themselves to maintain files in file cabinets. Consistently communicates information and ideas to others. Must be able to exchange accurate information in a manner others understand in these situations. Frequently observes details at close range (within a few feet of the observer). Occasionally moves boxes up to 20 pounds across the office for various purposes.
