House Call Clinician

The CAIPA@Home program brings comprehensive medical care directly to homebound individuals. We work closely with patients' providers in a collaborative model to ensure a seamless continuum of care, allowing individuals to thrive and age safely in their own homes.


We are seeking a dedicated Physician Assistant or Nurse Practitioner to join our expanding CAIPA@Home team as a House Call Clinician. In this rewarding role, you will provide vital in-home and telehealth consultations to our patients. This position requires in-home patient visits and will entail travel and autonomy with flexible scheduling. 


Job Responsibility

  • Conduct in-home visits and telehealth visits including medication review, in-home diagnostic testing, review of systems, and physical examinations
  • Identify opportunities to optimize quality of life, health promotion, home safety, and disease management
  • Provide chronic care management services including patient education, care coordination, health system navigation, and patient advocacy 
  • Coordinate treatment plans and care plans with patients’ providers
  • Participate in IDT rounds for comprehensive patient care discussions
  • Perform or order select home based diagnostic testing 
  • Are licensed medical professionals
  • Have Basic Life Support (BLS) certification
  • Thinking critically to address patient needs and provide the appropriate resources
  •  Comply with all HIPAA regulations and maintain active licenses and certifications
Requirements
  • Physician Assistant with a BS/MA degree from an ARC-PA accredited Physician Assistant Program or Nurse Practitioner with a Master level or above degree from an accredited Nurse Practitioner program
  • Current New York State Physician Assistant OR Nurse Practitioner License
  • Current BLS  certification
  • Bilingual in English and Chinese (Mandarin and/or Cantonese)
  • Experience in Gerontology, Internal Medicine
  • Home care or home  visit experience
  • Reliable transportation to travel to patient homes with provided medical equipment
  • Excellent written and verbal communication skills
  • Ability to work independently as well as part of a team

Salary: 

$120,000 - $170,000 inclusive productivity incentives
