Job Description:
As a Wellness Nurse in our adult outpatient program, you’ll provide creative, empathetic, patient-centered care in both office and community-based settings to support individuals living with severe and persistent mental illness. In this role, you’ll be an essential team member that supports our patients to gain and maintain holistic wellness by delivering medication administration and monitoring, patient and family education, and clinical assessment and intervention strategies! No two days are exactly the same—and that’s part of the excitement!

Key Responsibilities:
- Provide comprehensive psychiatric nursing services, including:
- Medication monitoring and administration including delivery of injectable medications and Clozaril observation.
- Offer patient and family education to empower understanding, insight, and long-term wellness.
- Provide ongoing clinical assessment including mental status updates and AIMS exams.
- Respond to patient crises with clinical skill to support safety and stabilization.
- Coordinate services with internal and external teams of therapists, case managers, prescribers, and peer support specialists.
- Contribute to treatment planning by providing observations and feedback in clinical treatment team settings.
- Champion recovery, stability, and continuity of care for individuals navigating complex behavioral health needs.
