Support Services Coordinator (Full Time/Tues, Wed, Thurs 8a-4p, Fri & Sat 11a-7p, flexibility as needed)
Description

This position is responsible for admission, assessments and orientation, carrying a caseload, and group facilitation. This position also includes some responsibility in assisting the Reintegration Manager with oversight of day-to-day responsibilities.


Essential Duties and Responsibilities

• Is responsible for the overall management of the service programming in conjunction with the Reintegration Manager, as well as carrying a small caseload.

• Assists the Reintegration Manager with the development of other program staff members.

• Oversees the maintenance of the program’s individual case records in compliance with agency policy and licensing authority regulations.

• Develops and assists other staff members with service planning and discharge planning.

• Facilitates weekly didactic and process groups as needed.

• Documents individual progress in accordance with agency policy and procedure.

• Meets with residents regularly to monitor progress and revises individual service plans as needed.

• Makes referrals, coordinates services, and maintains regular ongoing consultations with other providers.

• Coordinates educational and/or vocational skill development.

• Organizes and maintains case records.

• Assists with the admission process and orientation of new residents as needed.

• Provides breathalyzer and toxicology screening as needed.

• Responds to any crises and emergencies.

• Completes data input into the agency database.

• Responds in a professional and responsible manner to incidents involving contagious diseases (i.e., HIV, Hepatitis B, TB, etc.)

• Assists with the program’s medication policies and procedures.

• Is available for on-call responsibilities.

• Is available for 24-hour coverage when needed.

• Participate in weekly staff meetings and case conference.

• Serves on agency committees as requested.

• Carries out additional duties essential to the above-stated responsibilities as assigned.

• Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners.

• Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.

• Regular attendance at work location.

Requirements
  • Must possess and maintain an active Qualified Health Professional (QHP) CASAC-Advanced Status and have a Bachelor’s Degree in Human Services with at least 6 months’ experience in the substance use, mental health, or human services fields. 
  • Completion of OASAS Clinical Foundations Trainings 1 & 2  
  •  Valid New York State Driver’s License
Salary Description
$24.48/Hour
