Resident Support Services Manager

 What You’ll Do

  • Champion residents’ best interests and protect vulnerable individuals
     
  • Conduct psychosocial assessments and develop person-centered care plans
     
  • Collaborate with nurses, physicians, and interdisciplinary teams
     
  • Guide families through resources, referrals, and community services
     
  • Lead discharge planning to ensure smooth transitions home or to other care settings
     
  • Address behaviors proactively to enhance safety and reduce unnecessary medication use
     
  • Coordinate medication reviews to support gradual dose reduction
     
  • Serve as a liaison between residents, families, and community agencies
     
  • Maintain accurate, timely clinical documentation
     

What You Bring

  • Strong knowledge of social work practices, especially within geriatric populations
     
  • Excellent communication and organizational skills
     
  • A collaborative mindset and team-oriented approach
     
  • Ability to maintain strict confidentiality
     
  • Experience consulting with LCSW, LSW, or MSW professionals

Requirements

Education

  • Associates or Bachelor’s Degree in a psychology-related field, preferred.


Licenses/Certification

  • Must have a valid driver’s license.


Experience

  • Six months experience in a long-term care environment preferred.
