Overview:

Provide case management, advocacy, and support services to homeless individuals and their children who are participating in the supportive housing program. Ensure program success by supporting individuals and their children to create long-term goals, weekly goals, and objectives that lead to improved skills/education/income and movement to permanent housing or maintain permanent housing. Provide information and referrals, home visits, community meetings/programs and advocacy for participants with community providers for services. Maintain appropriate recordkeeping and tenant records in accordance with federal, state and Center City Housing Corporation requirements. Work closely with Site Director, Case Management Coordinator, Early Childhood Program Coordinator, Youth Program Coordinator, Case Managers, Property Manager, and other community agencies.

Duties and Responsibilities:

1. Conduct preliminary screening for potential clients.

2. Schedule, attend, and perform intakes with potential participants eligible for Housing.

3. Meet with new participants to review program rules, program expectations, and appropriate paperwork.

4. Provide direct services to a case load of 14-16 individuals and families residing in supportive housing.

5. Conduct weekly scheduled home visits/case management meetings with families residing in supportive housing.

6. Provide case management, guidance and support to resident families for impacts related to homelessness, domestic violence, and other adjustment difficulties or personal barriers.

7. Assess, model, monitor and refer life skills such as independent living, parenting, budgeting and home care, etc.

8. Assist clients in obtaining needed services such as employment, childcare, health care, substance abuse treatment, mental health treatment and transportation, etc.

9. Advocate with various systems, including but not limited to the police, schools, and courts.

10. Write weekly progress notes and case summaries. Maintain up-to-date confidential, written files on participants.

11. Complete monthly service updates, evaluation tools and other required reporting documents.

12. Minimize vacancies and keep units occupied in a timely manner.

13. Work with other case managers to facilitate groups, organize and co-facilitate monthly community meetings and other educational and community-building activities.

14. Attend weekly staff meetings.

15. Attend community agency meetings and assist the organization with building and maintaining its network of resources.

16. Support children’s programming by working in both TOTs and FUSION programs.

Requirements

Preferred Experience:

Bachelor’s degree in human service field preferably with two years experience in case management for homeless people OR Associate Degree in human service field with three years experience in case management for homeless people. This position requires a valid driver’s license and dependable transportation. This person must pass and maintain Minnesota Background Study clearance.

Essential Skills:

• Ability to read, analyze and complete documentation required by government agencies and communicate through speaking, listening and writing with residents and outside agencies.

• Demonstrate competency using computer programs such as Microsoft Word as well as ability to learn and use new systems as needed.

• Self-motivation and the ability to work independently including prioritizing job duties.

• Ability to maintain confidentiality.

• Understand and be sensitive to the effects of trauma on an individual and its impact on successful residency, including an understanding of homeless populations.

• Skill in understanding and maintaining positive relationships with diverse residents, applicants and co-workers.

• Regular attendance, reliability and punctuality

• Sensitivity and effectiveness when working with people from diverse backgrounds and with special needs.
