Responsibilities:

• Complete and oversee accurate and timely MDS assessments 
• Ensure compliance with state and federal regulations 
• Coordinate care planning process with interdisciplinary team 
• Monitor case mix, reimbursement, and documentation accuracy 
• Participate in audits and quality improvement initiatives 

Qualifications:

• Active LPN or RN license (Ohio) 
• Prior MDS experience required 
• Strong knowledge of Medicare/Medicaid regulations and reimbursement 
• Detail-oriented with strong organizational skills 
• Ability to work independently and as part of a team
