Description

PURPOSE STATEMENT


The Chronic Care Navigator implements, manages, and coordinates all Chronic Care Management Program activities, to include nursing triage, chronic care management, and care gap.  The navigator will engage, manage, and triage complex clinical needs.  Responsible for overseeing a team of Care Managers on a daily, weekly, and monthly basis to ensure top quality patient care and Chronic Care Management performance.


ESSENTIAL FUNCTIONS

  • Developing a proper understanding of chronic pain and primary care practice requirements for optimal, coordinated population health.
  • Proactive patient management and triage via text using multiple platforms.
  • Proactive patient management via phone calls.
  • Triage and assessment of patient health status.
  • Coordination of care between patients and providers.
  • Active review of patient charts to assess health needs and gaps.
  • Providing a positive patient experience for patients and families through courteous digital interactions.
  • Routing patient escalations accurately and expeditiously.
  • Making referrals to appropriate staff when necessary.
  • Maintain strict confidentiality.
  • Maintain regular and predictable attendance.
  • Perform other essential duties as assigned.
Requirements

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS 

  • High school diploma or equivalent.
  • LPN- graduate of an accredited Licensed Practical Nursing program and licensed to practice as a professional Licensed Practical Nurse (LPN) in the state of Missouri. Medical Assistant- current certification as a Certified Clinical Medical Assistant (CCMA), Certified Medical Assistant (CMA), or National Certified Medical Assistant (NCMA) is required or obtained within 90 days of hire.
  • 3 years’ primary care experience required.
  • Current AHA or Red Cross BLS.

PHYSICAL/MENTAL REQUIREMENTS 

  • Must be able to sit and stand, intermittent 8 to 10 hours a day.
  • Must be able to use standard office equipment, including the telephone and computer keyboard.
  • Continuously works under pressure of near 100% accuracy while meeting inflexible deadlines.
  • Continuously utilizes manual/bi-manual dexterity, near vision, speech, and hearing.
  • Frequently stands, walks, sits and utilizes eye/hand coordination and color definition.
  • Occasionally reaches above shoulder, regularly required to lift and/or carry up to 40 lbs.
  • Occasionally walks on uneven surfaces.
