About the Role


Greater Nashua Mental Health is seeking a Mental Health Court Case Manager to provide evidence-based case management services to justice-involved adults with serious mental health and substance use disorders. This role supports participants in meeting court requirements, accessing treatment, and building long-term stability and recovery while serving as a liaison between the court, legal partners, and treatment providers.


Key Responsibilities


• Complete intake and ongoing case management assessments and develop individualized service plans.

• Coordinate and monitor participant engagement in appropriate levels of care.

• Track and report participant progress and compliance with Mental Health Court requirements.

• Address key recovery needs including housing, transportation, employment, and community supports.

• Connect participants to medical, educational, social, and community resources.

• Provide direct and indirect case management services.

• Facilitate individual and group Enhanced Illness Management and Recovery (EIMR) services, including leading at least one weekly group.

• Attend weekly supervision, Mental Health Court staff meetings, and assigned court hearings.

• Serve as liaison with courts and legal partners, sharing treatment updates as appropriate.

• Maintain timely documentation within 3 business days and ensure required releases of information are current.


Qualifications


• Bachelor’s degree in psychology, social work, criminal justice, or a related field.

• Knowledge of mental health conditions and substance use disorders.

• Familiarity with criminal justice–involved populations and the court system.

• Knowledge of city and state community resources.

• Strong communication, organizational, and collaboration skills.

• Able to work independently and as part of an integrated team.

• Comfortable in a fast-paced environment and managing multiple priorities.

• Proficiency in Microsoft Office (Word, Excel, Outlook).

Salary Range: $21.92/hr - $27.02/hr
