Position Purpose 

The Care Navigator is a remote, member-facing role centered on providing personalized guidance and support to members and their family as they navigate their behavioral health resources. In this role, the Care Navigator serves as a knowledgeable, compassionate guide - helping members understand their behavioral health options, identify barriers to care, and clarify their personal goals. From there, the Care Navigator collaborates with members to develop and carry out a plan that meaningfully supports their overall well-being. 


Duties & Responsibilities  

Client Engagement and Follow-up 

  • Guide members through their available resources - educating them on their options and offering personalized recommendations that support confident, informed decision-making. 
  • Deliver tailored follow-up to members with prior interactions and proactively re-engage those with inconsistent participation.  
  • Help members and their families navigate VITAL WorkLife benefits and, when appropriate, transition to insurance-based care, including verification support 
  • Educate members on their confidentiality rights and maintain transparency throughout the course of care 
  • Serve as a subject matter expert for VITAL WorkLife’s digital platforms, guiding members through the mobile app, website, and additional tools 
  • Administer intake conversations for members seeking coaching support and coordinate administrative logistics following Physician Peer Coaching intakes 
  • Collaborate with consultants to ensure a seamless, connected member experience 

Client Satisfaction Support 

  • Partner with the WBS Manager to prioritize and triage service issues, ensuring members receive timely attention and resolution 
  • Collaborate cross-functionally with the Clinical Team and other VITAL departments to ensure appropriate levels of care 
  • Respond to satisfaction reporting comments with a one-business-day resolution standard 
  • Collect post-session data for Physician Peer Coaching and contribute insights on member trends and market needs 

Well-Being Coordination  

  • Answer and triage member calls and inquiries with thorough documentation and case management 
  • Coordinate authorizations through VITAL's nationwide consultant network 
  • Maintain professional, compassionate communication with members, organizations, consultants, and internal teams 
  • Contribute to quarterly projects and annual department goals in collaboration with your peers and manager

Requirements

Qualifications

  • Bachelor's degree in social work, healthcare administration, nursing, or a related field or equivalent experience  
  • 2–3 years of experience in care coordination, case management, or client advocacy 
  • Proficiency in Motivational Interviewing 
  • Knowledge of healthcare systems, medical terminology, and community resources 
  • Strong communication, interpersonal, and organizational skills with the ability to work both independently and collaboratively 
  • Professional conduct, empathy, and cultural sensitivity when working with diverse client populations 
  • Familiarity with HIPAA and a commitment to safeguarding confidential information 
  • Proficiency in electronic health records and Microsoft Office Suite 

Remote Work Requirements 

This position is fully remote. To ensure a seamless, private, and professional experience for every client, candidates must have a dedicated private workspace and a reliable high-speed internet connection. These are conditions of employment for this role.

Salary Description
$20-$25/hour
