Pathways Hospice and Palliative Care is seeking a dedicated and experienced Registered Nurse (RN) to join our leadership team as a Patient Care Manager (PCM). This role is ideal for a nurse who is passionate about end-of-life care and ready to support and lead a five-star nursing team in delivering exceptional hospice services.
Position Summary:
The Patient Care Manager (PCM) provides day-to-day clinical leadership and support to hospice field staff, ensuring high-quality, patient-centered care. This role works closely with the Director of Clinical Services and interdisciplinary team to promote excellence in care delivery, compliance, and team engagement.
Shift Availability:
- Monday through Friday, with minimal administrative on-call responsibilities
What You’ll Do:
- Supervise and support a team of hospice RNs, LPNs, and CNAs
- Serve as a clinical resource for patient care, symptom management, and family support
- Oversee daily operations including visit coordination and caseload management
- Ensure timely, accurate documentation and regulatory compliance
- Participate in and help lead Interdisciplinary Group (IDG) meetings
- Provide coaching, mentoring, and performance feedback to staff
- Assist with admissions, recertifications, and complex patient needs as needed

- Active RN license in the state of Missouri
- Minimum 2 years of hospice experience
- Previous leadership, charge nurse, or preceptor experience strongly preferred
- Strong clinical judgment and communication skills
- Ability to thrive in a fast-paced, team-oriented environment
- Comfortable using electronic medical records and technology
