Rapid Rehousing Case Manager
Description

 This position is responsible for creating positive outcomes for clients through our Rapid Re-housing programs’ operations, delivering direct support services, and collaborating with internal staff and community providers to achieve client goals.

Responsibilities: (to be performed with or without reasonable accommodation):

  • Operate within the DuPagePads Core Values, which supports providing care in accordance with the Philosophy of Care, Trauma Informed Policy and Practices of the Agency.

· Oversee and manage all files, documentation, program and auditing requirements for DuPagePads rapid rehousing programs.

· Travel throughout DuPage County to meet with housing clients in their homes, in community settings, and/ or office locations.

  • Teach clients independent living skills through instruction, modeling, coaching, and direct support.
  • Act as client advocate to facilitate access to resources and services.
  • Work collaboratively with providers to achieve client goals; liaise with community resources on behalf of clients.
  • Develop and maintain client goal plans, regular assessments, and complete other required documentation.
  • Complete and thorough documentation of client contact, progress report, case files, and contractual paperwork.
  • Maintain collaborative relationships with colleagues to effectively support clients.
  • Collaborate with team members to provide support groups and workshops to achieve participants goals.
  • Maintain confidentiality of privileged information and adhere to client privacy laws; demonstrate sensitivity of diverse ethnic groups, genders, and cultures.
  • Act as housing liaison between clients and landlords, attending to housing issues, coordinating internal and external moves for residents.
  • Arrange transportation for program participants to activities/appointments on a regular basis.
  • Participate in required training, client staffing, supervision, department, and agency meetings.
  • Adhere to agency policies regarding attendance and punctuality.
  • Provide after-hours phone coverage and response during weeknights and weekend while scheduled for on-call.
  • Flexibility in scheduling to accommodate client and program coverage needs.
  • Other duties as assigned.

Qualifications:

  • Associate degree in human services or related field or relevant knowledge, experiences, or skillset (preferred)
  • Experience working with client populations, conducting home visits, or in housing-related activities a plus.

Other:

  • Flexible work hours may be required.
  • Be able to lift 10 pounds.
  • Reliable transportation and possess a valid Illinois Driver’s License.

 
 

Salary Description
$21-22 per hour
