POSITION: Licensed Professional Counselor/Social Worker
DEPARTMENT: Medical Services STATUS: Exempt
REPORTS TO: Director of Clinical Care
Position Summary:
Serves as a primary care Behavioral Health Consultant team member providing consultation for patients and their primary care providers within the collaborative care model
DUTIES AND RESPONSIBILITIES:
- Develops, implements, reviews, and regularly improves Clinical Treatment Plans to ensure compliance with all relevant regulations and best practices
- Discharge planning & manages facility relationships with aftercare providers to ensure each patient is set up for continued success in his or her recovery journey
- Biopsychosocial Evaluations and Individualized Treatment Plans; understands chemical dependency and
- Participates in Treatment Team meetings to evaluate progress of all patients and make recommendations for adjustments to Treatment Plan
- Conducts individual patient therapy sessions, provides brief, same-day consultations and crisis intervention as needed
- Provides direct support to clinic medical staff
- Conducts trauma informed care and Human Trafficking care
- Utilizes behavioral health screening tools as part of patient care (e.g., PHQ-9, GAD-7, PSC-17, Social Determinants of Health)
- Conduct CANS Assessment; conducts other psychosocial assessments, as needed
- Records and maintains progress notes for all encounters
- Conducts appropriate discharge of patients and makes appropriate referrals for patients, including psychiatric
- Provides traditional individual, family or group psychotherapy services, as needed
- Performs case management duties as necessary
- Participates in case staffing and administrative meetings with clinical supervision and seeks peer consultation as needed
15. Ensures delivery of medical care in accordance with the Catholic Moral and Ethical Directives
16. Follows all HIPAA, OSHA, and licensing guidelines, regulations and code of ethics
17. Maintains confidentiality of patient information/records at all times
18. Maintains established San José Clinic policies, procedures, objectives, quality assurance, safety, environmental and infection control
19. Implements job responsibilities in a manner that is consistent with the San José Clinic Mission and Code of Conduct and is supportive of San José Clinic cultural diversity objectives
Qualifications, Education, and Experience:
- LPC/LCSW – licensed in the State of Texas and functions within the Code of Ethics
- Master’s degree in counseling psychology, counseling, Social Work or equivalent field, preferred
- Minimum 5 years’ experience in a clinical role within Chemical Dependency or Human Trafficking field with a minimum of 2 years managerial experience
- At least 2 - 4 years of experience in the medical field or within an integrated primary care setting, working with behavioral health issues related to chronic disease management, depression, anxiety and trauma
- Must be committed to drug-free lifestyle and responsible alcohol use
- Must have previous counseling experience with strong clinical skills with adults, children, and adolescents and knowledge and experience in family systems
- Must have strong clinical and assessment skills, cultural competence and basic casework skills with referrals and advocacy
- Demonstrates excellent organizational skills, multi-tasking and effective use of time
- Able to handle and complete multiple tasks or projects with multiple deadlines
- Able to deal professionally, courteously and efficiently with public and all levels of the organization
- Able to operate computers, scanners, and copier faxes
- Must be proficient in Microsoft Office and practice management system software application
- Demonstrates effective verbal and written communication skills
- Demonstrates effective customer relation skills, working with diverse populations, often in stressful fast paced environments
15. Bilingual in English/Spanish, required

- Ability to sit, stand, bend and stoop for (long) periods of time
- Ability to exert up to 50 pounds of force occasionally/ frequently
