Service Area
The RN will provide care in the following areas:
- Skokie
- Rogers Park
- Edgewater
- Lincoln Square
- Lake View
- Lincoln Park
- Downtown Chicago
- Surrounding areas
Responsibilities
- Completing the nursing process in the delivery of patient care, including assessment, planning, implementation, and evaluation of patient outcomes
- Total oversight of patients from admission to discharge
- Developing a plan of care based on findings
- Serving as the patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care
- Timely completion of all skilled nursing documentation
- Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
- Working within a multidisciplinary team approach, coordinating with PT, OT, ST, MSW, HHA’s, and office staff

Requirements
- Current Illinois State RN License
- CPR Certification
- Previous experience in nursing or other healthcare field experience
- Familiarity with medical software and equipment
- Ability to build rapport with patients
- Strong problem-solving and critical-thinking skills
- Ability to thrive in a fast-paced environment
- WellSky EMR knowledge is preferred but not required
Salary Description
$65-$75/per visit, commensurate with experience
