We are seeking an extraordinary, energetic, and relationship-driven Registered Nurse Care Coordinator to join our dedicated team. This role is vital in ensuring we meet the needs and expectations of our clients and their families, while also strengthening our presence within the community.
Position Overview
The RN Care Coordinator / Case Manager plays a key role in client assessments, care planning, quality assurance, and community relationship development. This position blends clinical oversight with customer service and community engagement.
Key Responsibilities
Assessments & Care Planning
- Conduct initial in-home assessments for prospective clients
- Develop individualized care plans based on client needs and family expectations
- Ensure all care plans align with non-medical home care scope and safety standards
Client Experience & Quality Assurance
- Perform routine supervisory visits for current clients
- Monitor quality of care and ensure consistency in service delivery
- Update care plans as client conditions or needs change
- Collaborate with families, physicians, and other providers for continuity of care
Community Outreach & Relationship Building
- Build and maintain relationships with referral sources and community partners
- Serve as a resource for clients, families, and community organizations
- Represent Visiting Angels in the local senior care network
Caregiver Support & Engagement
- Provide guidance and training to caregivers on client-specific needs
- Ensure caregivers feel supported, valued, and set up for success
- Promote strong communication between caregivers and the office team

Compensation & Benefits
- Hourly Pay: $40–$45/hour (DOE)
- Quarterly Performance Bonus Eligibility
- Paid Time Off (PTO) – accrued even as part-time staff

Schedule Details
- 24- 40 hours per week (3-5 days a week)
- Flexibility required to meet client and community needs
- Work conducted throughout the Twin Cities area

Qualifications
Required:
- Active Registered Nurse (RN) license in the State of Minnesota
- Strong communication and interpersonal skills
- Excellent organizational and time management abilities
- Ability to multitask and prioritize in a fast-paced environment
- High attention to detail in documentation and follow-up
Preferred (but not required):
- Case management experience
- Sales, outreach, or relationship development experience
- Experience in home care or senior services
