Schedule: Full-time with day, evening and weekend shifts available


Salary: $20.00


Sign On Bonus-up to $1,000

About ACHIEVE's Residence Program: In the residential program, the most caring people are the trained staff who provide 24-hour supervision, care, and guidance to residents. They assist with personal care needs, cleaning, cooking, medications, and activities. The Direct Support Professional - Residential provides self-care training and assistance in daily living to individuals with developmental disabilities residing in a group home setting.


DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Provides individualized care as outlined in the individuals Life Plan, or ISP.
  • Assists Individuals in the primary activities of daily living (toileting, feeding, cooking, cleaning) as outlined in individual's ISP to promote independence.
  • Maintains files and produces service notes and documentation on assigned caseload.
  • Documents and submits reports to the program as outlined in the assigned department.
  • Model appropriate social skills to assist the individual in developing and maintaining relationships in the home and in the community
  • Assists Individuals in the development of natural supports within the community.
  • Attends all established or assigned in-services and web-based training.
  • Provides First Aid, CPR, and SCIP-R as needed.
  • Uses behavior management techniques as directed in the individual behavior plans and/or medication monitoring plans. 
  • Give medications as prescribed by physician, per OPWDD regulations
Requirements
  • High school diploma or GED; Valid NYS driver's license and 12 months driving history. Six months to one-year related experience and or training preferred.
