Position Summary
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our clinical team. The LPN provides high-quality nursing care, administers medications and treatments, monitors resident conditions, and collaborates with the interdisciplinary team to support positive resident outcomes.
Key Responsibilities
- Administer medications and treatments as ordered.
- Monitor residents and promptly communicate changes in condition.
- Provide direct nursing care and support activities of daily living as needed.
- Collaborate with physicians, nursing staff, and the interdisciplinary team on resident care plans.
- Maintain accurate and timely clinical documentation.
- Communicate effectively with residents, families, and healthcare providers.
- Promote a safe, respectful, and resident-centered care environment.
Requirements
Qualifications & Requirements
- Current, unencumbered West Virginia LPN license.
- Skilled nursing or long-term care experience preferred.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently and collaboratively as part of a care team.
- Compassionate and resident-focused approach to care.
- Ability to meet the physical requirements of the position, including lifting/moving 50 pounds or more and standing or walking for extended periods.
