SUMMARY  

The Director of Clinical Partnerships is responsible for strengthening, protecting, and expanding Alive’s most critical referral partnerships within an assigned market. This role serves as market-facing leader who aligns community relationships, internal teams, and evolving service offerings to support sustainable growth, earlier engagement, and long-term partnership stability.  


ESSENTIAL DUTIES AND RESPONSIBILITIES 

1. Own and steward Alive’s strategic referral partnerships within the assigned market, with particular focus on high-priority (strategic) accounts.  

2. Develop and execute market-level partnership strategies aligned with organizational goals and territory admission targets.  

3. Lead market-level account prioritization, including A/B/C tiering.  

4. Serve as the primary market partner to Alive Clinical Directors, ensuring alignment on priorities, expectations, and escalation pathways.  

5. Collaborate closely with the Access Center, Intake, and Admissions teams to support timely referral response, visibility, and service recovery.  

6. Step in to support at-risk or deteriorating strategic partnerships and lead service recovery efforts as needed.  

7. Support the introduction and adoption of new and innovative service lines in collaboration with clinical and operational leaders.  

8. Monitor market performance trends, identify risks or opportunities, and recommend course corrections.

• Strength and stability of strategic referral partnerships within the market.  

• Consistent contribution to territory admission targets and reduced volatility.  

• Improved market predictability and reduced referral leakage.  

• Successful integration of new or innovative service lines with minimal disruption.  

• Positive partnership feedback from clinical leaders and community partners.  

• Reduced need for escalation over time as systems and relationships stabilize.  


QUALIFICATIONS & EXPERIENCE  

• Demonstrated experience building and sustaining complex referral or partnership relationships in healthcare.  

• Expert in hospice services and community-based care environments preferred.   

• Ability to lead through influence and collaboration rather than authority.  

• Comfort navigating ambiguity, change, and evolving service models.
