As an Enterprise Practice Optimization Manager (EPOM) you will: 

• Operates under a train-the-trainer model to support deployment of the Practice Optimization Program (POP) in market. 

• POP includes activities related to best practices, workflows and process improvement, application use of technologies to optimize patient care, practice productivity, revenue and other value-based, population health management efforts. 

• An EPOM is responsible for thorough, effective training of in-market practice optimization teams to ensure success in practice optimization efforts. EPOMs serve as support for in-market practice optimization teams. When necessary and prior to establishing in-market Practice Optimization teams, an EPOM is responsible for providing high quality, customer focused onsite field support for physician practices. Support provided is tailored to meet practice needs and promote practical, meaningful and sustainable changes. An EPOM is responsible for providing continuous input based on learnings and insights gathered from client settings. An EPOM builds strong relationships with physician practices, Navvis clients, and partner agencies to support, promote and execute the Navvis vision, Navvis client vision and physician practice visions. 


A Day in the Life:

• Train in-market practice optimization teams to ensure successful deployment of practice optimization program

• Support and as appropriate, augment, in-market practice optimization teams

• When supporting physician practices:

      o Deploy Practice Optimization Program which optimizes processes, people and technology (fundamentals including pre-

                visit planning processes, daily huddles, care team, etc. and process improvement activities, effective use of technology)

      o Provide input on POP based on physician practice interactions

      o Identify core motivation of practice members and create energy around practice advancement

      o Identify and link common goals and visions between physician practices, the Navvis client and Navvis

      o Clearly and frequently communicate with practices and clients regarding high impact information

      o Promote and facilitate clear communication within the practice and between the practice, partner agencies and Navvis          

                clients

      o Create and maintain strong relationships with physician practices

      o Inspire practice leadership and practice members to be engaged and take ownership of metrics and data collection


What Success Looks Like in this Role:

• Maintain a deep understanding of Navvis’ client measures and requirements

• Identify, assess, and collaboratively work to resolve short-term and long-term issues

• Ensure buy-in from key stakeholders and those affected by changes




Requirements

We are excited about you if you have these things:

• BS or BA degree in Business or Healthcare related field or equivalent experience

• 3+ years working directly with physicians and practice members 

• HCC/RAF knowledge preferred

• PCMH knowledge preferred

• Demonstrated ability to work effectively as a member of an interdisciplinary team, displaying good judgement, problem solving, and decision-making skills

• Excellent oral and written communication skills; able to communicate professionally and effectively to a diverse audience

• Strong organizational and analytic skills to gather, organize, research and report data necessary for performance improvement

• Ability to manage stressful situations

• Ability to de-escalate situations

• Self-directed and highly motivated

• Ability to travel to various locations both within the state and outside the state of Florida

• Proficiency with Microsoft Office 365, productivity software, including but not limited to Word, Outlook, PowerPoint, Excel, Lists, Skype meetings and SharePoint

• Proficient in population health management technology platforms
