Under the direction of the Manager of Case Management, the Social Worker is responsible for assessing, identifying, formulating, and implementing an appropriate and safe plan of discharge. This is accomplished through Psychosocial assessments, interventions, and collaboration with the hospital care team and community agencies/facilities.

· Must have a Bachelor’s Degree in Social Work or related field

· Experience in hospital social work is preferred

· Knowledge of community resources

· Highly motivated, assertive, computer skills necessary, knowledgeable in community resources and alternate care facilities, demonstrates exceptional written and verbal communication skills

· Ohio License or license eligible

· BLS upon hire and maintain current
