Description

Job Responsibilities & Functions:

  • Provides direct patient care and administers medication to the terminally ill patient and performs related tasks at SH, per the plan of treatment as assigned by the case manager
  • Provides personal care, assistance with activities of daily living, homemaking/housekeeping, and related assistance to patients to increase the level of comfort, and to maintain personal hygiene and a safe, healthy environment for the patient and co-workers
  • Communicates effectively and timely with supervising nurse and other hospice aides by performing a change of shift rounds
  • Takes and records patient vital signs and reports changes in patients’ condition and/or needs to RN
  • Changes simple dressings or ostomy bags as directed
  • Performs blood sugar finger sticks and documents results and notifies RN of abnormal results
  • Answers call bells promptly to respond to patient/family questions or concerns
  • Administers and monitors the supply of medications within the scope of practice per certification
  • Documents and reports residents’ response to medication if adverse according to Coastal Hospice policy and procedure
  • Collaborates as a team member to meet patients’ plan of care
  • Maintain good personal hygiene and project a professional image
  • Participates in weekend/holiday rotation to meet patient needs


Requirements

Education and Work Experience Requirements:


  • Possess a high school degree or equivalent and be eighteen years of age
  • Possess a current certification to practice as a Certified Nursing Assistant with the Board of Nursing in Maryland (MBON)
  • Possess valid/current Certified Medication Technician certification with MBON
  • Current CPR certification for Healthcare Provider (BLS)
  • Ability to read and follow written instructions and document care given
  • Ability to use a computer and other office equipment to document patient information
  • Ability to communicate verbally and in writing clearly with individuals and families in end-of-life situations
  • Demonstrated ability to make meaningful patient observations and write brief, accurate reports for patient files
  • Self-directed with the ability to work with limited supervision
  • Ability to be flexible and able to work on several projects simultaneously
  • Ability to work harmoniously with many different types of individuals and groups


Other Requirements:

  • Advance knowledge and skills; documenting 12 hours of in-service training per year
  • Must be able to lift and carry up to 50 pounds with or without the use of assistive devices
