The CUA Child Welfare Outcome Specialist works to support the Case Management Team in helping families achieve Single Case Plan goals and move towards safe, timely case closure. This position involves working closely with all members of the assigned case management team; with children, biological families, and resource families; and with external agencies and partners. Tasks will include direct work with families to address safety threats and diminished protective capacities, as well as work with external partners and systems to ensure that children and families have the support they need in their communities to achieve the goals and objectives for each family
Key job tasks/duties/responsibilities of the CUA Child Welfare Outcome Specialis
1. Function as part of the larger CUA team responsible for achieving the target goals of improving safety, permanency, and well-being of children, youth, and families.
2. Provide home and community life skills education and intervention services to children, youth, and their caregivers toward ensuring family stabilization, reunification, or other permanency.
3. Work with parents and caregivers who may have cognitive limitations, physical or emotional disabilities, or any combination of these.
4. Participate in Family Team Conferences when necessary.
5. Attend Court Hearings as needed but never in lieu of the CUA Case Manager.
6. Document any activity they have performed within six business days in the appropriate section of ECMS unless subcontracted in which case using the CUA-prescribed form.
7. Conduct regularly scheduled visits to assigned families and to establish and maintain a trusting relationship with families using a strengths-based approach.
8. Meet with the supervisor to evaluate family status, parent-child progress and to discuss strategies for improving outcomes.
9. Complete all paper work in compliance with program requirements.
10. Work as part of the CUA Support Team toward achieving the objectives as established in the Single Case Plan (SCP).
11. Facilitate safe case closure for in home service cases.
12. Facilitate reunification or other permanency by:
a. Assisting the CUA CM to focus on permanency opportunities.
b. Tracking and managing the movement of cases through the permanency process.
c. Assisting the CUA CM in preparing the children, youth, and families for reunification or other permanency.
d. Supporting the CUA CM in preparation for Court.
13. Supervise visits when necessary.
14. Assist with transportation when necessary
15. Completing referrals and follow-ups as needed.
16. Attend staff meetings in order to contribute to program issues and update on agency issues.
17. Attend scheduled in-service training in order to develop professional skills.
18. Provision of on-call services in cases of emergency and on a rotational basis as required by APM CUA protocol
19. Perform other duties that support the mission of APM and the CUA program.
