As an Adult Case Manager at Pine Tree Society, you will provide resources and support to meet the individualized needs of individuals with intellectual disabilities, Pervasive Developmental Disabilities and Autism Spectrum Disorders.
Our team assists in the development, coordination and facilitation of a comprehensive person-centered plan (PCP) designed to meet the specific needs of the people we serve. We emphasize community inclusion and empower individuals and their families to be as independent as possible while ensuring they receive the support they need.
