Iowa City Hospice is adding a PRN Social Worker position to assist our team in providing social work support as well as covering other positions within our psycho social departments for leave coverage (vacation, FML, open positions....).

Iowa Care Community provides foundational support for nonprofit home health care organizations, taking care of human resources, billing, marketing, fundraising and more. It will provide care coverage in the office setting as well as in patient home settings that will require driving from location to location within our seven county service territory.

Our Social Workers, bereavement counselors and volunteer coordinator providing psycho social support to patients and families from time of admission up to and through bereavement needs of the patient’s survivors.   This PRN position will support a variety of patient needs and associated tasks within our Psychosocial department.

Schedule: Monday through Friday, 8am - 5pm; we can talk about flexible schedules based on business needs.


Essential Functions of this position:

  • Conduct a comprehensive assessment of the patient including a psycho social assessment to evaluate the patient/caregiver’s emotional, social, financial, and environmental resources and identify appropriate psycho social problems, interventions and goals for the patient’s plan of care.
  • Participate as a member of the Iowa City Hospice Interdisciplinary Team in the development and implementation of the patient’s plan of care.
  • Provide direct social work services and interventions according to the patient’s plan of care.
  • assist the patient/caregiver with advance directives;
  • identify the patient/caregiver’s psycho social needs;
  • assess and strengthening the patient/caregiver’s coping skills
  • assess and enhancing the responsiveness of the environment and connecting the patient/caregiver with community resources as needed.
  • provide interventions for specific symptom relief (such as fear, grief, depression, anger, etc)
  • screen for psychopathology and providing education and interventions accordingly;
  • enhance the strengths of the family system;
  • assess and refer for bereavement services;
  • provide assistance with housing, welfare and safety issues as needed;
  • help the patient/caregiver to access financial, legal or other services as needed;
  • document problems, assessment finds, goals, car provided, interventions and patient/caregiver response to care;
  • maintain the dignity of the dying patient; support the patient/caregiver’s spiritual and cultural beliefs;
  • provide holistic, family-centered care across treatment settings; consult and collaborate the interdisciplinary team and care partners;
  • report suspected cases of abuse or neglect; conduct family conferences as needed
  • assist with funeral planning as needed.
  • Reassess the patient/caregiver needs every 15 days and, with the Interdisciplinary Team, update the patient’s plan of care.
  • Documents all care provided in the patient’s clinical record in accordance with Iowa City Hospice’s documentation requirements.
  • Serve as an educational source to staff, care partners and community.
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
  • Participate in Iowa City Hospice’s success by ensuring access to our exceptional care.
  • Support and promote Iowa City Hospice’s mission to lead in providing exceptional patient-centered end-of-life care and grief support to anyone affected by a terminal illness.
  • Participate in the organization’s quality assessment and performance improvement program.
  • Ensure effective use of resources.
  • Perform other duties as assigned.
Requirements
  • Current Iowa license as a Social Worker is preferred.
  • Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education.
  • 1 year of social work experience in a health care setting
  • Monday – Friday 8am-5pm based on work related demands; with no guaranteed hours per pay period. PRN supports coverage needs during schedule leave and requires "availability" to work a minimum of 4 scheduled shifts per pay period. 
  • Ability to communicate effectively in the English language both verbally and in writing
  • US work authorization (Required)
  • Passionate about home care with a genuine desire to help others
  • All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
  • Reliable, professional and able to start directly after the completion of background check
  • 2 step TB testing is required.
  • First Aid training
  • Mandatory Reporter Training
  • Iowa Driver License, Car, adequate liability insurance and current driver’s license are required. Clients’ homes are located throughout seven counties (fifty mile radius of the agency) the driving requirements include in-town and highway driving in good and inclement weather.
  • COVID-19 considerations: COVID 19 Vaccination or medical/religious exemption required; applicable infection control standards are applied company wide to assure a safer work environment.
