Description

INTERNAL MEDICINE PHARMACIST

  • Works collaboratively with multidisciplinary teams to optimize drug therapy.
  • Provide direct patient care, including treatment assessment and medication management activities.
  • Clinically evaluate all patients in a specified area on a daily basis with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.
  • Review the patient’s drug regimen for an appropriate indication, dosing, allergy verification, interactions potential, lab monitoring, duplication of therapy, drug interactions and medical documentation. Provide cost-effective management of drug utilization within the clinic. Monitor process and outcome of patient care relating to drug therapy.
  • Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.

 

Requirements

INTERNAL MEDICINE DEPARTMENT PHARMACISTS – A minimum of one-year pharmacy experience within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency or equivalent experience is required. Board certification is preferred. Chief, Department of Pharmacy has the option to waive requirements.
