The Targeted Case Manager helps clients build stability, independence, and long-term success in recovery. This position supports Isaiah House’s mission by connecting clients with needed services, resources, and support. Through strong planning, teamwork, advocacy, and follow-up, this role helps improve client outcomes and supports quality care.

Additional Information:
- Schedule: Full-time, Monday–Friday, 8:00 AM–5:00 PM
- Work Environment: On-site
- Travel Requirements: To other facilities if support is needed

Key Job Responsibilities:
- Complete client assessments and create individualized care plans.
- Coordinate services with internal teams and outside providers.
- Help clients access medical, social, educational, vocational, and community resources.
- Advocate for clients and support informed treatment decisions.
- Maintain accurate documentation while following KAR standards and company policies.
Required Qualifications
- Bachelor’s degree in behavioral science, healthcare, human services, or a related field.
- Minimum of one year of post-graduate experience in a healthcare or human services-related field.
- Strong written and verbal communication skills.
- Excellent organization, time management, and problem-solving skills.
- Ability to multitask, work as part of a team, and follow established processes.
Preferred Qualifications
- Experience as a Targeted Case Manager in the substance use disorder field.
- Working knowledge of case management principles, healthcare management, and reimbursement.
- Medicaid knowledge.
- Current TCM CORE/SUD certification.
