Sangre de Cristo Community Care is seeking a Part-Time Registered Nurse to join our dynamic Home Health team in Cañon City, Colorado. This role offers the opportunity to provide high-quality, compassionate care while working alongside a supportive, mission-driven team dedicated to serving communities across Southern Colorado.
Schedule & Pay
- Part-time position
- 3 days per week | 8 hours per day
- Starting pay: $35–$38/hour (based on experience)
About the Role
As a Home Health Registered Nurse, you will play a vital role in coordinating and delivering patient-centered care in the home setting. You’ll use your clinical expertise and judgment to support patients, families, and the interdisciplinary team in achieving the best possible outcomes.
Key Responsibilities
- Plan, coordinate, implement, and evaluate nursing care for assigned patients
- Assess patient needs and coordinate services and referrals to support individualized care plans
- Collaborate closely with the interdisciplinary team to ensure continuous, comprehensive, and responsive care

- A valid non-restricted RN license in the state of Colorado is required.
- Must have a valid non-restricted driver’s license.
- Flexibility and availability to take call for home health and hospice patients.
- At least 1 year of home health or hospice experience preferred.
