What You'll Do

• Serve as the trusted point of contact for concierge clients and their families.
• Coordinate personalized care plans, appointments, transportation, and community resources.
• Develop workflows, service standards, and operating procedures for the Concierge Program.
• Collaborate with caregivers, clinical staff, and community providers to ensure seamless care.
• Monitor client satisfaction and identify opportunities to elevate the client experience.
• Partner with leadership on program growth, quality initiatives, and new service offerings.
• Support quality assurance, compliance, documentation, and operational excellence.

What You'll Bring

• Current NYS Registered Professional Nurse (RN) license. (required)
• Home care experience preferred. 
• Supervisory experience preferred.
• Knowledge of RN, LPN, PCA, and paraprofessional scope of practice.
• Proven leadership ability, with experience coaching and developing clinical teams.
• Exceptional customer-service, communication, and interpersonal skills.
• Entrepreneurial mindset with the ability to build programs, create workflows, and operationalize new initiatives.
• Familiarity with EMRs, Microsoft Office Suite, and modern communication tools.
• Valid NYS driver’s license and reliable transportation.
