Position Summary
The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more.
You’ll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.
What You’ll Do
- Build strong, therapeutic relationships with clients—including those who may be harder to engage
- Help clients set and achieve personalized wellness goals
- Provide skill-building and psychoeducational support (individual & group settings)
- Coordinate care plans and connect clients to essential community resources
- Support clients and families in making informed decisions
- Assist with access to medical, behavioral health, and social services
- Provide crisis intervention when needed
- Maintain accurate documentation and meet service delivery standards

- Experience with behavioral health, case management, or human services preferred
Work Environment
This is a dynamic, community-based role requiring travel to client homes, service sites, and community locations. Work may be conducted in-person or via telehealth, depending on client needs.
Schedule
- Full-time, Monday–Friday (8:00 AM – 5:00 PM)
- Occasional evenings or extended hours as needed
Physical Requirements
- Ability to sit, stand, walk, and use standard office equipment
- Occasionally lift up to 25 lbs
- Travel within the service area required
