Are you passionate about making a meaningful difference in people’s lives? Join our team and help empower individuals with long-term psychiatric disabilities to thrive in their communities. We are seeking a Full Time Adult case manager for our Presque Isle Outpatient Office. 


Position Summary

The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more.

You’ll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.


What You’ll Do

  • Build strong, therapeutic relationships with clients—including those who may be harder to engage 
  • Help clients set and achieve personalized wellness goals 
  • Provide skill-building and psychoeducational support (individual & group settings) 
  • Coordinate care plans and connect clients to essential community resources 
  • Support clients and families in making informed decisions 
  • Assist with access to medical, behavioral health, and social services 
  • Provide crisis intervention when needed 
  • Maintain accurate documentation and meet service delivery standards

  • Flexibility and willingness to meet clients where they are (including community-based work) 
  • Experience with behavioral health, case management, or human services preferred 

Work Environment

This is a dynamic, community-based role requiring travel to client homes, service sites, and community locations. Work may be conducted in-person or via telehealth, depending on client needs.


Schedule

  • Full-time, Monday–Friday (8:00 AM – 5:00 PM) 
  • Occasional evenings or extended hours as needed 

Physical Requirements

  • Ability to sit, stand, walk, and use standard office equipment 
  • Occasionally lift up to 25 lbs 
  • Travel within the service area required

Requirements

  

  • The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field. 
  • Bachelor’s degree preferred. Must be certified at the full MHRT-C or provisional level. Use of personal vehicle and active driver’s license is required.
