Description

Join our team as a Care Coordinator in a hybrid, full-time role where you can make a meaningful impact in the lives of individuals and families. 


As a key member of our metro team, you will play a vital role in ensuring clients receive timely, coordinated, and person-centered care that supports their overall well-being. 


In this position, you will assess client needs through intake and preliminary screenings, connect individuals and families to appropriate services and community resources, and collaborate with internal and external partners to develop and implement effective treatment plans. You will help address Social Determinants of Health (SDOH), support referral management and scheduling, and maintain accurate, timely documentation within electronic client record systems. Additionally, you will participate in multidisciplinary team meetings, monitor Encounter Alert Service (EAS) notifications and follow up on hospital or emergency department discharges, and ensure ongoing communication across provider caseloads to promote continuity of care. This role also includes participating in rotation weeks for new client inquiries and referrals, assisting with annual paperwork and releases of information, and completing other related duties as assigned. 

Requirements

To be employed as a Care Coordinator, an individual must meet all the qualifications:

MINIMUM QUALIFICATIONS:

  • High School Graduate or GED
  • Individual must be non-judgmental in regards to lifestyle, race and/or economic status.
  • Must be able to pass a DHS background study and meet applicable requirements and regulations.
  • Must be at least 21 years old and have a valid Minnesota Driver’s License.

PREFERRED EXPERIENCE:

  • Experience and knowledge in behavioral health programming.
  • 2 years of college in a healthcare or human service related field or 3 years of experience preferably working within a behavioral health clinic or social services.

  • Excellent client center care, interpersonal and organizational skills

  • Knowledgeable about community resources

  • Ability to use Electronic Medical Record (EMR) system

  • Able to professionally collaborate with internal and external providers and agencies.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.

Salary Description
$20.00 to $24.00 per hour
