We are seeking a dedicated and experienced Certified Nursing Assistant who will provide direct care to residents, while promoting the resident’s highest level of functioning, as directed by management. Additionally, they will assist in maintaining the resident’s environment in a safe and neat manner, as well as promoting quality services within community standards, State and Federal Regulations.

ESSENTIAL JOB FUNCTIONS:
• Supports the mission, vision, and goals of the organization, upholding and promoting company culture and vision.
• Receives and communicates report to management on resident status at the beginning and end of each shift and as needed. Notifies the management of any changes in resident medical condition/status.
• Provides personal care to residents per their individualized plan of care.
• Documents all pertinent information regarding care as assigned.
• Prepares residents for meals and assists to and from the dining areas. Assists with set up and feeding of meals.
• Assists residents with toilet and incontinent care. Makes incontinent checks at least every 2 hours and provides incontinent care as needed.
• Makes room checks and verifies resident location at least every 2 hours.
• Keeps resident’s rooms neat and clean.
• Performs clinical procedures according to community policy.
• Performs vital signs, B/Ps, weights and other trained procedures as directed by management.
• Completes prevention and restorative care that includes but not limited to PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL care.
• Maintains a working knowledge of Community policy & procedure pertinent to role.
• Assists in turning, positioning, and repositioning of residents.
• Participates actively in Interdisciplinary Team Process including completing assessments and documentation as assigned. Attends care plan meetings as assigned. Acts as resident advocate.
• Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplines.
• Actively participates in Community Quality Assurance Performance Improvement Program.
• Maintains education development to include attending ongoing education development to maintain compliance with State and Federal Regulations and current knowledge of resident care practices.

EXPERIENCE, QUALIFICATIONS & SKILLS:
• Certification of completing an approved Certified Nursing Assistant is required.
• Six months’ experience in long-term care is preferred.
• Successful completion of State C.N.A. course.
• Make standard and routine decisions based on detailed guidelines with little independent judgment. Problems are generally solved with clear, detailed guidelines or by reporting them to a supervisor.
• Must have a basic level of skill and can follow written or verbal instructions.
• Must have basic technical skills to complete a few repetitive and well-defined duties.
• Must have a basic knowledge of the organization to answer basic questions.
• Must have working knowledge of facility emergency procedures including fire and disaster drills.
• Ability to follow facility policies.
• Ability to maintain resident confidentiality and adhere to HIPAA policies.
PHYSICAL DEMANDS: The following physical demands are representative of those that must be met to successfully perform the essential functions of this position:
• Walk/Stand – must be able to continuously walk and stand.
• Environment Condition – must be able to perform work both inside and outside.
• Sit – sit infrequently.
• Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.
• Carry - constantly 0-10 pounds, frequently 11 – 50 pounds.
• Push - constantly 0-10 pounds, frequently 11 – 50 pounds.
• Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available.
• Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may require frequently manipulating weights of 25 pounds or more.
• Squat – must be able to squat on a frequent basis to reach below the knee, may require lifting of 50 pounds or less.
• Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more.
• Reach– must be able to reach on a frequent basis. May occasionally reach overhead, requiring manipulation of weight of 10 pounds or less, and below waist manipulating weight of 25 pounds or more.
WORKING CONDITIONS: • Required to interact with co-workers, residents or vendors. • Occasional weekend, evening or night work if needed to ensure shift coverage. • Possible exposure to communicable diseases and infections. • Potential injury from transferring, repositioning, or lifting residents. • Exposure to latex. • Possible exposure to blood-borne pathogens. • Possible exposure to various drugs, chemical, infectious, or biological hazards. • Subject to injury from falls, burns, odors, or cuts from equipment.
