Position Summary

The Community RN provides clinical oversight and medical management for individuals served in Community Living residential programs. This role is responsible for developing, implementing, and monitoring individualized health care plans while ensuring compliance with Missouri DMH standards, physician orders, and nursing best practices.

The RN works independently across multiple residential homes to ensure safe, coordinated, and person-centered medical care that supports health, safety, and quality of life.


Qualifications

  • Active Missouri RN license required
  • Bachelor’s degree in Nursing required
  • CPR/First Aid certification required
  • 2+ years of nursing experience preferred
  • Experience in developmental disabilities or community-based care preferred
  • Strong clinical assessment, documentation, and critical thinking skills
  • Knowledge of DMH/DD regulations and nursing standards
  • Ability to work independently across multiple sites
  • Strong communication, training, and leadership skills
  • Proficiency with Microsoft Office and electronic documentation systems

Key Responsibilities

Clinical Oversight & Care

  • Conduct nursing assessments and develop health care plans
  • Monitor medical conditions, vitals, labs, and health trends
  • Review physician orders, MARs, and medical documentation
  • Assess medication effectiveness and side effects
  • Ensure timely follow-up on medical changes and incidents

Delegation & Staff Support

  • Delegate and oversee nursing tasks per Missouri Nurse Practice Act
  • Train and monitor staff on medication administration and health supports
  • Ensure staff competency and compliance with delegated responsibilities
  • Provide L1MA training and ongoing oversight

Compliance & Quality Assurance

  • Audit medical records and ensure regulatory compliance
  • Maintain DMH, Medicaid waiver, and licensing standards
  • Monitor and follow up on incidents, hospitalizations, and ER visits
  • Support audit readiness and documentation accuracy

Care Coordination

  • Collaborate with physicians, specialists, pharmacies, and providers
  • Communicate with families regarding medical updates and care plans
  • Support appointments and continuity of care
  • Participate in interdisciplinary team meetings

Team Collaboration & Operations

  • Partner with clinical, operations, and direct support teams
  • Participate in on-call rotation as needed
  • Complete required documentation, reporting, and training
  • Support additional nursing or operational needs as assigned

 Starting Salary: Our compensation is competitive within the market. The starting rate for this position begins at $75,000 annually, with your final offer based on a combination of your education, relevant experience, and overall qualifications as well participation in BHA's bonus program.
