LIFE Senior Services is seeking a compassionate, driven Independent Case Manager (ICM) to support older adults and individuals with disabilities in maintaining independence and quality of life. This vital role combines direct client support with community resource coordination to ensure individuals can safely remain in their homes and communities.
Position Overview
As an Independent Case Manager, you will serve in a dual capacity:
- SeniorLine Support: Provide information, assistance, and referrals to individuals through phone, email, or in-person interactions.
- Case Management Services: Work collaboratively with clients to assess needs, develop care plans, and coordinate services that promote independence and well-being.
Key Responsibilities
Client Support & Intake
- Provide courteous, compassionate, and active listening to assess client needs
- Offer accurate information and referrals to community-based services
- Assist individuals in navigating available programs, options, and eligibility requirements
- Maintain accurate and timely documentation in the SeniorLine database
Case Management
- Conduct comprehensive client assessments
- Develop and implement individualized, person-centered care plans
- Coordinate and monitor services to ensure quality, cost-effective outcomes
- Advocate for clients to support independence and community integration
- Facilitate communication among clients, families, service providers, and agencies
Program Support & Collaboration
- Maintain up-to-date knowledge of community resources
- Assist with program enrollments, including Adult Day Health (ADvantage) services
- Participate in community outreach and engagement activities
- Serve as a liaison with internal teams and external organizations
Compliance & Documentation
- Maintain accurate and timely client records in accordance with program policies
- Ensure compliance with HIPAA/HITECH regulations
- Participate in CARF accreditation processes
- Complete required training and continuing education

Requirements
Qualifications & Experience
- Bachelor’s degree preferred in Social Work, Psychology, or a related behavioral science field (or equivalent experience)
- Experience working with older adults and/or individuals with disabilities
- Knowledge of community resources serving these populations
Skills & Competencies
- Strong problem-solving and critical thinking abilities
- Excellent organizational skills with the ability to manage a caseload effectively
- Ability to meet strict deadlines while maintaining accuracy and compliance
- Strong interpersonal skills with the ability to build rapport across diverse populations
- Ability to work both independently and collaboratively as part of a team
- Proficiency with computers and data entry/reporting systems
Physical Requirements
- Frequent sitting, standing, and walking throughout the workday
- Repetitive use of hands, including grasping and fine finger dexterity
- Occasional bending, reaching (including overhead), squatting, and kneeling
- Ability to communicate clearly with strong English comprehension
Salary Description
$40,500-$45,000
