General Purpose:
Under the supervision of the Director of Health Home Care Management, the Program Supervisor is responsible for oversight of care management, outreach, and for conducting outreach activities to individuals with chronic illnesses, including HIV, for the purpose of engaging them into the Cornerstone Health Home Program. The Program Supervisor will complete enrollments with eligible members in their home and/or in clinical settings, including hospital ERs and clinics. The Program Supervisor is also responsible for establishing and maintaining collaborative relationships with hospitals and medical providers, to facilitate ongoing referrals from them, and will serve as the point of contact for co-location projects. The Program Supervisor will directly supervise Care Management staff, ensuring that program outcomes are met. The Program Supervisor will provide quality oversight of client charts in the EHR and ensure adherence to billing standards for care management. The Program Supervisor will carry a caseload as needed, and perform all the responsibilities of a Care Manager during times of staffing shortages or transitions.
Key Competencies:
- Engage HH outreach clients by conducting outreach services each month, and completing all necessary documentation for enrollment into Health Homes.
- Meet and maintain program billing and productivity standards as required in the Electronic Health Records.
- Establish and maintain collaborative relationships with community partners and providers, to facilitate ongoing referrals from them, and represent Cornerstone and the Health Home Program, in various community settings.
- Provide training and supervision to Health Home staff, to support professional development and adherence to program policies, procedures, and best practices.
- Facilitate growth of the Health Home program through enrollment and retention opportunities as well as maximizing billing of appropriate services.
Description of Duties:
- Engage HH outreach clients into service by conducting outreach services each month, which can include mailing letters, internet searches, cold calls, and field visits to last known address or hospitals/clinics.
- Conduct brief health assessments on potential clients to see if they meet the chronic condition criteria necessary for Health Home enrollment. Conduct Comprehensive Assessments, following enrollment
- Work with the Lead Health Home to establish workflows and oversee implementation of co-location opportunities.
- Establish and maintain collaborative relationships with hospitals and medical providers to facilitate ongoing referrals from them.
- Serve as the point person for co-location projects, in the community. Coordinate all aspects of community co-location projects, including scheduling, workflows, and data monitoring.
- Attend Health Home meetings and provide Cornerstone Health Home updates, as applicable.
- Facilitate growth of the Health Home program by exploring opportunities to enroll and retain members.
- Meet and maintain program billing and productivity standards.
- Maintain Electronic Health Record and all required electronic data and complete all program standards documentation as required.
- Train new and existing CM staff.
- Complete EHR and paper chart audits to ensure charting compliance.
- Provide weekly supervision to CM staff, and other staff as assigned.
- Provide Case Management services to an assigned caseload, on an as-needed basis.
- Participate in supervision and program review each week with the VP of Health Homes.
- Provide supportive training to all direct reports and department staff as necessary.
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with Cornerstone policies and procedures and the Employee Handbook.
- Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other related duties as assigned.
Education/Experience Required:
- Master’s degree in social work, counseling or related field. Some experience in human services and experience in a supervisory position or BSW or equivalent degree with 1-3 years of experience in a supervisory position.
- Must be well organized; outstanding oral and written communication skills; ability to manage time and multiple tasks efficiently; familiarity with non-profit and/or human service agency operation, sensitivity to HIV/AIDS issues and ability to work independently.
- Must be proficient in Microsoft Word and Excel. Experience with electronic data entry is a plus. Bilingual (English/Spanish) is a plus. Some flexibility to work early evening hours.
- The Program Supervisor may work periodically throughout the week in an Emergency Room setting. Must be able to provide immunization records for purpose of working with clients in hospitals/clinical settings.
